The latter two tools require that you monitor at a node where all flows or a significant number of flows can be monitored (in Cisco, a SPAN port or MONITOR port). Once you have determined the nature of it, you can then take steps to reduce the problem (bandwidth limits on the switch port, Quality of Service adjustments, Queue adjustments). Keep in mind that protocol options can make a BIG difference -- HTTP "Keep Alive" for instance has a huge impact on HTTPS secure pages when they have many objects (without keepalive, each object will generate an entirely new SSL negotiation, an enormous amount of traffic and some carriers will kill SYNC packets above a certain rate).
